QLINICARE TERMS & CONDITIONS

These Terms & Conditions (“Terms”) govern your use of Qlinicare services. By accessing or using our website, booking appointments, or receiving care, you agree to these Terms.
1. ELIGIBILITY (NO PEDIATRIC SERVICES)
Qlinicare provides services only to individuals 18 years of age or older.
We do not provide care to minors under any circumstances, including with parental or guardian consent. Any appointment booked for a person under 18 will be canceled without notice.
By using our services, you confirm that you are at least 18 years old.
2. SERVICES PROVIDED
Qlinicare offers telehealth-based medical services, including but not limited to:
- Primary care services
- Acute care visits
- Chronic disease management
- Medical weight management
All services are provided by a licensed healthcare professional and are limited to conditions appropriate for telehealth.
Qlinicare reserves the right to determine whether a condition is appropriate for virtual care.
3. NO EMERGENCY SERVICES
Qlinicare does not provide emergency or urgent care services.
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
4. TELEHEALTH LIMITATIONS
By using Qlinicare, you understand that:
- Care is delivered via video, phone, or electronic communication
- A physical examination may be limited or not performed
- Diagnosis and treatment rely on the accuracy of the information you provide
- Technical issues may impact the quality of care
You accept these limitations when choosing telehealth services.
5. PATIENT RESPONSIBILITIES
You agree to:
- Provide accurate, complete, and truthful information
- Be physically located in a state where the provider is licensed (if applicable)
- Be available and on time for appointments
- Follow medical advice and treatment plans
Qlinicare is not responsible for outcomes resulting from incomplete or inaccurate information.
6. APPOINTMENTS, CANCELLATIONS & NO-SHOWS
Appointments must be scheduled in advance.
As part of Qlinicare’s membership model, there are no fees for missed appointments or late cancellations.
However, patients are expected to attend scheduled visits on time. Repeated missed appointments or no-shows may result in:
- Temporary scheduling limitations, or
- Requirement to reschedule based on availability
Qlinicare reserves the right to manage scheduling access to ensure fair availability for all members.
7. PAYMENT & FEES
Qlinicare operates on a self-pay and/or membership model.
- Qlinicare operates on a transparent membership model with no hidden fees.
- Membership fees are billed on a recurring basis (monthly unless otherwise stated)
- Membership includes access to services as outlined and does not include medications, labs, or external services unless specified
- There are no additional fees for missed appointments, cancellations, or rescheduling
- Membership fees are non-refundable once billed
- Failure to maintain an active membership may result in limited access to services or even cancellation of services.
8. MEMBERSHIP MODEL
Qlinicare may offer a recurring membership program.
By enrolling, you agree:
- Membership fees are billed on a recurring monthly basis
- Membership includes only the services explicitly listed
- Membership does not include medications, labs, or external services unless stated
- Fees are non-refundable once billed
- You may cancel at any time; cancellation takes effect at the end of the billing cycle
- Qlinicare may modify pricing or structure with notice
Failure to maintain an active membership may limit access to care.
9. WEIGHT MANAGEMENT SERVICES
Qlinicare offers medical weight management services, which may include:
- Lifestyle and nutritional guidance
- Behavioral support
- Prescription medications (when appropriate)
You understand that:
- Results are not guaranteed and vary by individual
- Success depends on adherence to the treatment plan
- Medications may have risks and side effects
- Ongoing monitoring may be required
Certain medications, including GLP-1 therapies, are prescribed only when medically appropriate and at the provider’s discretion.
10. PRESCRIPTIONS
Prescriptions may be issued based on clinical judgment.
- Qlinicare does not guarantee prescriptions
- Controlled substances may not be prescribed
- Pharmacies may have their own policies affecting fulfillment
11. PRIVACY
Your information is handled in accordance with our Privacy Policy and applicable laws, including HIPAA.
12. LIMITATION OF LIABILITY
To the fullest extent permitted by law, Qlinicare is not liable for:
- Technical issues or service interruptions
- Delays in care due to incomplete information
- Patient non-compliance with medical advice
- Outcomes beyond reasonable clinical control
All services are provided “as is” without guarantees of specific results.
13. THIRD-PARTY SERVICES
Qlinicare may use third-party platforms (e.g., scheduling, video, payment processing).
We are not responsible for the performance or policies of third-party services.
14. TERMINATION OF SERVICES
Qlinicare reserves the right to deny or terminate services at any time for:
- Misuse of services
- Non-compliance with policies
- Safety or legal concerns
15. MODIFICATIONS
Qlinicare may update these Terms at any time. Continued use of services constitutes acceptance of any changes.
16. GOVERNING LAW
These Terms are governed by the laws of the State of Florida.
